Doctors’ Choice Accepts Most PPO Insurance And Some HMO Plans

To help you with your billing, Doctors’ Choice offers:
Direct Billing to Your Insurance Provider
Doctors’ Choice will gladly check your benefits with your insurance provider so you will know up front what portion they will cover and what portion will fall under the patients’ responsibility. We will bill your insurance company on your behalf for the urgent care services that we provide. Once we bill your insurance and hear back from them, we’ll send you a statement that details the remaining balance. You’ll also receive an Explanation of Benefits (EOB) from your insurance company showing you the amount billed to them, what portion they have covered, and your remaining responsibility.
Cash Pay Patients
For patients who may not have medical insurance coverage Doctors’ Choice does accept cash patients. Our medical staff will access what services need to be provided, and will be able to let you know what the cost of those services will be. The balance owed is due at the time of service. If you have special circumstances requiring special payment arrangements please let our staff know you would like to speak with a manager.

Please Provide Us With All Of Your Insurance Info When You Visit
We’ll need a copy of your insurance card and your driver’s license. If you are not the subscriber of the insurance that we will bill for your services then please provide us with the subscriber’s social security number and date of birth. If your insurance information has changed since your last visit then let us know.
Understanding Your Insurance Plan’s Benefits & Requirements
Most insurers require you to make a co-pay at the time of the visit and pay any amount that’s not covered by your plan. That amount varies by plan and depends on if how much of your deductible you’ve met this year. Please contact your insurance provider if you have any questions – the number is on the back of your insurance card.
Respond Promptly To Statements From Doctors’ Choice And Your Insurer
When you receive statements from Doctors’ Choice and your insurance company, please review them carefully and pay them promptly. If you feel there are any errors, let us know and we’ll be happy to review your statement with you.
